Our Lady of Perpetual Help Catholic School uses an online application process.
There is a $60 non-refundable application fee that will be collected before
submission of the application.
All applicants must submit a copy of:
- Birth certificate
- Catholic baptismal certificate (if applicable)
- First Communion certificate (if applicable)
- Most recent report card (grades 1-8)
- Most recent standardized test results (grades 1-8)
Applications without required documents are considered incomplete and will NOT be
presented to the admissions team for review until submitted.
Once your application has been submitted, the applicant will be invited to complete a
grade-appropriate entrance assessment.
All new students are admitted to OLPH on a sixty-day probationary basis. This allows
parents, teachers, and administrators to evaluate the student’s opportunity for success.
OLPH will not discriminate based on race, color, sex, national or ethnic origin. OLPH
welcomes all denominations when space is available. All students enrolled at OLPH
will participate in religion class and attend weekly Mass.
Students entering Pre-Kindergarten (PK) must turn 4 on or before August 31 of entry
year.
Students entering Kindergarten must turn 5 on or before August 31 of entry year.
